Sometimes they have a blue tick, sometimes they are followed by thousands and thousands. It's usually pretty clear. Another method is see how many genuine accounts follow them.

Best way I find to find fakes is they often follow and talk to loads of hot teenagers! Haha. The old fake Sean Long one was the best for that (has a real one now!).
